Most failures fall into four categories: stale indexed data, provider unavailability, changed onchain prerequisites, or wallet/network mismatch. Preserve the distinction between unknown, failed, and merely delayed.
Refresh the specific name, listing, endpoint, policy, job, or fleet evidence rather than the entire browser session.
Confirm the active wallet and chain before repeating a transaction.
Treat simulation reverts as evidence to fix a prerequisite, not a prompt to force-send.
For partial batches, inspect each item result and retry only the unresolved items.
Use block-explorer evidence and get_transaction_status for submitted transactions.

Common symptoms
Name missing from portfolio — check owner versus manager role, wrapper state, wallet, and index freshness.
Portfolio is still building — wait for the timestamped snapshot state instead of treating missing rows as zero holdings.
Calendar feed is stale — check the calendar client refresh interval, then rotate or recreate a revoked feed.
Saved-search match looks wrong — compare the stored normalized rule with the live name or listing state.
Inbox action is stale — open its destination and refresh current evidence before signing anything.
Market pulse is degraded — unavailable metrics should display as unknown; inspect the named source before interpreting a zero.
Listing cannot fill — re-check owner, order expiry, counter, signature, and token approval.
Record update reverts — check manager authority, resolver compatibility, wrapper path, and calldata target.
Agent shows unknown — no fresh stored observation is not the same as a failed endpoint.
Policy fails — inspect signature, issuer, revocation, expiry, exact tool, suffix, resolved grantee, and value.
Commerce is not escrowed — acceptance alone is escrow-ready; funded evidence must match every field.
Fleet is partial — inspect the named worker dimension; other workers remain independently usable.
